Telehealth Consent and Acknowledgment

This page provides general information about telehealth and does not replace the informed-consent document presented by your treating licensed provider. If you proceed with a telehealth visit, the provider’s consent, privacy notice, clinical policies, and applicable state law control.

What telehealth involves

Telehealth may involve communication with a licensed provider by secure video, audio, messaging, or another remote technology. The provider will explain the nature of the visit, answer questions, and determine whether telehealth is appropriate for your circumstances.

Benefits and limitations

Potential benefits may include convenience and access to a provider. Telehealth also has limitations. Technical interruptions, incomplete information, the absence of an in-person examination, or your location may affect whether a provider can evaluate or treat you remotely. A provider may recommend in-person care, additional evaluation, or referral at any time.

Privacy and participation

You should participate from a private location when possible, use a secure device and internet connection, and tell the provider if anyone else is present. The provider will identify any other participant in the visit and address required consent. You may withdraw from a telehealth encounter and ask about alternatives, although doing so may affect the provider’s ability to evaluate you.

Emergencies and location

Telehealth is not for emergencies. If you have an emergency, call 911 or seek immediate in-person care. Before and during a visit, you may be asked for your physical location and a way to contact you in case urgent help is needed.

Clinical independence

Evolve BioRx does not provide medical care. Any diagnosis, treatment decision, prescription, or referral is made solely by the independently licensed provider who conducts the clinical assessment. Treatment availability and provider licensure vary by state and individual circumstances.
